How do I obtain a quote for my company?
  In order for us to submit your company's request for a benefits quote, the Group Census  
  Form
needs to be completed and returned to our office. Click here to download

We would like to compare rates and benefits from different insurance companies. Can you send me a comparison chart?
  Yes. Our agency has excellent relationships with top-rated insurance carriers and their
  distribution channels. We can provide your company an illustration chart so you can select
  a benefits plan that will meet your company's financial objectives and at the same time,
  help you create a well-designed employee benefits package for your employees.

Our company is interested in certain insurance carriers only. Can you still provide a quote?
  Yes. Please indicate your choices on the Group Census Form and we can provide a quote
  with your preferred carriers and plans.

What is the effective date of coverage?
  Depending on the date your group application is received, carriers assign either the 1st or
  the 15th of the month as the effective date of your group's coverage.

What is AB 1672?
  AB 1672 is an important law that was passed in 1992 to help small business owners and
  their employees obtain health insurance, maintain long-term coverage, and enable
  employees to change jobs without losing their coverage. AB 1672 provides four major
  protections: Guaranteed Issue, Guaranteed Renewal, Premium Protection and Portability
  for Employees.

What is the employee participation requirement?
  The participation requirement varies depending on the insurance company.

Can an eligible employee waive participation in the company's health plan?
  Yes. If the eligible employee has coverage elsewhere, such as his or her spouse's
  employer, that employee may waive coverage. Insurance companies may require that the
  employee complete and sign a waiver form.

How much is the employer contribution?
  The employer contribution varies depending on the insurance company. Traditional
  contribution ranges from 50% to 100% of the cost. Some employers contribute a Fixed
  Dollar Amount which could range from $100 or more per employee per month.

Can my company offer multiple plans/carriers?
  Yes. Please ask your Benefits Representative for carrier guidelines.

What does RAF stands for?
  RAF stands for Risk Adjustment Factor. For each product, the insurance company sets a
  standard rate for each age range, ZIP code, and number of people covered such as
  employee, employee and spouse and family. Insurance companies can adjust rate by the
  Risk Adjustment Factor - so a group's rate may be higher or lower than the standard rate.

Can you briefly describe the enrollment process?
  Once your company decides on the types of plan you wish to offer your employees,
  Bozzuto Benefits Insurance Services will provide assistance during the entire enrollment
  process. We will supply your employees with the application forms, guide you through the
  process ensuring that all paperwork are complete, and make sure that participation and
  contribution guidelines are met.

How long does it take to process our group application?
  The approval of your group application may vary depending on the insurance carrier but
  you can be assured that our agency will advocate for a prompt approval of your case.
  Bozzuto Benefits Insurance Services and its partners will �scrub� your group application
  to make sure that all underwriting requirements are met.

Our company has a new hire - what do I do?
  The employer establishes its eligibility guideline during the application process. Once a
  new hire satisfies the eligibility period, the employer can offer the group plan to the
  employee. An employee application is required.

Our renewal date is coming and we need to review our benefits plan - can you assist our group?
  The best time to review your current benefits plan is on or before your open enrollment
  period. This allows our insurance professionals to work with you in determining if your
  benefits plan still meets your company and employees' needs. We will conduct a thorough
  review and analysis and make recommendations as necessary.

Who should I call for claims and other administrative issues?
